Mr. Wang has received radiotherapy for nasopharyngeal carcinoma for nearly 10 years, but he still has many relapses. One day, suddenly, a large amount of blood gushed from the mouth and nose, with a total volume of more than 2000 ml, and I was immediately shocked. He was immediately sent to the hospital for rescue. What is the root cause
This “fatal” epistaxis. It is the most dangerous type of epistaxis. The root of the bleeding is caused by the rupture of the internal carotid artery, but the causes of the “rupture” are different. Generally speaking, the common causes of internal carotid artery “rupture” are as follows:
① Head trauma. It is often caused by car accidents or falling from a high place, resulting in skull fractures, damage to the arterial wall and formation of pseudoaneurysms. The tumor body gradually grows. Under certain incentives, such as hard breath holding, cold, fever, etc., bleeding can occur. Master Dai is in this situation.
② Congenital or acquired factors. Such as cerebral atherosclerosis, resulting in weak arterial wall and local expansion to form an aneurysm.
③ Radiation injury. For example, repeated radiotherapy in the skull base area damaged the arterial tissue, resulting in softening and necrosis of the internal carotid artery wall, which is often seen in patients with advanced nasopharyngeal carcinoma who have received multiple radiotherapy.
First signs
As mentioned above, the softening, necrosis or aneurysm of the internal carotid artery wall is like a bomb buried in the head, which may detonate at any time. However, before and after the onset of this epistaxis, there will be some premonitory symptoms, such as the “whoosh” sound emitted by lighting the fuse. If you can “smell” the source and cut off the fuse in time before detonating, a life-threatening disaster can be avoided.
